    Theory of microphase separation in multiple segment-type statistical multiblock copolymers with arbitrary block molecular weight distributions

    A Landau free energy is derived for the weak segregation regime (WSR) of melts belonging to a very general class of statistical multiblock copolymers, referred to as “multiple segment-type statistical multiblock copolymers.” Copolymer chains in this class consist of sequences of up to M⩾2 chemically different types of segments, organized into sequences of blocks of varying lengths (molecular weights). The possible sequences of blocks that are encountered in the copolymer chains, as far as their type is concerned, are described by a first-order Markov process, while the block molecular weight distributions of these M types of blocks are completely arbitrary. The number of blocks per chain is assumed to be large. This class of copolymers is sufficiently general to encompass all industrial relevant bulk statistical multiblock copolymers, such as all known thermoplastic elastomers. The particular free energy considered is just one realization of an even more general Landau free energy which is applicable to the WSR of melts of all conceivable copolymers, including homopolymers and all possible blends.     The perceived waning of biologics in severe asthma

    BACKGROUND: Biologics are highly effective in severe asthma and used at fixed dosing intervals. However, in clinical practice, dosing intervals are sometimes shortened if patients perceive a decreased biologic effect before the next administration. The occurrence and clinical relevance of this perceived waning of biological effect is unknown.OBJECTIVE: To explore (1) the frequency, severity and conditions, (2) associated symptoms and (3) relationship with clinical characteristics of the patient-perceived waning effect of biologics before the next administration.METHODS: Severe asthma patients receiving biological treatment ≥4 months were included. Based on 17 semi-structured patient interviews, we developed a questionnaire focusing on the waning effect of biologics before the next administration, which was distributed among 129 patients. Clinical characteristics, including asthma control (ACQ) and quality of life (AQLQ) scores, were collected from patient files.RESULTS: 65/101 patients who completed the questionnaire reported a waning of biological effect, graded as severe (median (IQR) 6.5 (5-7.5) on a 0-10 BORG-scale). Waning manifested in a broad spectrum of symptoms. Patients reporting waning had higher ACQ and lower AQLQ scores versus those without (p &lt; 0.05) and higher BORG-scores were associated with higher exacerbation rate (ρ = 0.309, p = 0.013). A third of all patients were in favor of extending or shortening their dosing interval.CONCLUSION: Two-thirds of severe asthma patients report waning of biologic effect at the end of the dosing interval, which is associated with poorer asthma control and quality of life. The diversity in observed waning of effect opens the way for research into more individualized dosing of biologics.</p

    Support in Assessment of Prior Learning: Personal or Online?

    Assessment of Prior Learning (APL) offers significant benefits to adult learners. It reduces the gap between educational programmes and thelabour market and provides learners the possibility to shorten their prospective study programmes. APL however requires adequate support of the learners that engage in APL. This study investigates which support possibilities could be useful and efficient for APL candidates in a distance education context. Staff members of an APL provider (APL tutors and educational scientists), APL candidates and a group of experts on online support evaluated and discussed the possibilities of both embedded and personal support in APL. With regard to the different phases that can be distinguished in APL, the results show that all participants expect support particularly in the phase of gathering appropriate evidence. From the staff members’ perspective, embedded support is most appropriate and many types of support provisions are recommended. APL candidates prefer a combination of embedded and personal support, whereby the type of personal support can be limited to telephone and email support. The latter is preferred because of its personal, to-the-point and time independent character. An overview of the highest added value of support as well as support efficiency is provided. Unfortunately, the highest added value is not always the most efficient. Guidelines for the elaboration of efficient support with high added value in APL in the context of distance education are presented

    Self-assembly in solution of a reversible comb-shaped supramolecular polymer

    We report a single step synthesis of a polyisobutene with a bis-urea moiety in the middle of the chain. In low polarity solvents, this polymer self-assembles by hydrogen bonding to form a combshaped polymer with a central hydrogen bonded backbone and polyisobutene arms. The comb backbone can be reversibly broken, and consequently, its length can be tuned by changing the solvent, the concentration or the temperature. Moreover, we have proved that the bulkiness of the side-chains have a strong influence on both the self-assembly pattern and the length of the backbone. Finally, the density of arms can be reduced, by simply mixing with a low molar mass bis-urea

    A homozygous variant in growth and differentiation factor 2(GDF2)may cause lymphatic dysplasia with hydrothorax and nonimmune hydrops fetalis

    The etiology of nonimmune hydrops fetalis is extensive and includes genetic disorders. We describe a term-born female neonate with late onset extensive nonimmune hydrops, that is, polyhydramnios, edema, and congenital bilateral chylothorax. This newborn was successfully treated with repetitive thoracocentesis, total parenteral feeding, octreotide intravenously and finally surgical pleurodesis and corticosteroids. A genetic cause seemed plausible as the maternal history revealed a fatal nonimmune hydrops fetalis. A homozygous truncating variant inGDF2(c.451C>T, p.(Arg151*)) was detected with exome sequencing. Genetic analysis of tissue obtained from the deceased fetal sibling revealed the same homozygous variant. The parents and two healthy siblings were heterozygous for theGDF2variant. Skin and lung biopsies in the index patient, as well as the revised lung biopsy of the deceased fetal sibling, showed lymphatic dysplasia and lymphangiectasia. To the best of our knowledge, this is the first report of an association between a homozygous variant inGDF2with lymphatic dysplasia, hydrothorax and nonimmune hydrops fetalis
